Request Meals
Who can receive a meal from Senior Connect?
Individuals 60 years old or older
Residing within the service area
Primarily home-bound or have little or no help
Income is not a criteria for service. All people who receive home delivered are asked to make a voluntary contribution to share in the costs of the meals. No one is ever refused service because of inability to contribute.
To receive a home delivered meal, a case worker conducts an in-home assessment with the home-bound senior. Each impairment in activities of daily living and instrumental activities of daily living are quantified. Based on the State’s assessment form determining physical condition and need, these clients may become a part of a daily delivery route within days of in-home assessment.
To get started:
Call 469-376-MEAL (6325) or fill out the form below to make a referral. We will ask for the individual’s name, address, phone number, date of birth, an emergency contact and a brief description of what condition defines the individual as “homebound” (mobility, chronic condition, etc.)
Our case-worker will call and schedule an appointment to visit the referred individual in their home.
Once approved, meals will begin.
